Consumer Direct Care Network provides in-home care to people across South Dakota, from the Black Hills to the Big Sioux River. Our goal is simple: help people stay in their homes, keep their independence, and make their own choices about their care.

Consumer Direct Care Network South Dakota (CDSD) is proud to serve as a Family Support 360 provider for the South Dakota Division of Developmental Disabilities (DDD).

We are dedicated to supporting individuals and families across the state with personalized, self‑directed care. With a long history of empowering people to manage their own services and maintain independence in their homes and communities, CDSD is committed to delivering the same high‑quality support statewide.

Individuals receiving services through the Family Support 360 program are encouraged to visit our website for ongoing information and helpful resources.
